High Volume, Personalised Telemonitoring of Remote Patients

Telemonitoring of patients by hospitals, combined with a social way of patient self-management, is critical to alleviate over-burdened health systems around the world. This is specifically true for urban areas, where tens of millions of people live close together. The current medical IT infrastructure is not equipped to telemonitor massive volumes of patients in parallel. This requires high performance platforms, smart data management and embedded, expert-defined, easy to configure, personalised decision support, coaching and notification, on a high- performance dedicated IT infrastructure and business services platform. The objective of this project is to develop and validate a cross functional, generic, set of services that is able to address this need on a global scale, as an export product, with a focus on large cities and urban areas.
